Arbovirus heart disease: myocarditis and cardiomyopathy following dengue and chikungunya fever-a follow-up study

Resumo: Thirty-five patients with heart disease after arbovirus infections (dengue and chikungunya fevers) and referred to as "arbovirus heart disease" were studied. There were 17 males and 18 females, ranging in age from 5 to 58 years. The main clinical features, ECG's, chest radiographs, virological investigations, and follow-up studies are reported. Virological investigations showed evidence of arbovirus infection suggesting a cause-and-effect relationship. Virological investigations carried out on a control group of patients admitted to the same unit were negative. Follow-up studies showed that arbovirus infections affecting the heart carry a definite mortality rate and often result in permanent heart damage. Three patients died, one in the acute phase, and two within 6 months. Six patients recovered, four completely and two with residual ECG abnormalities. Twenty-six patients developed cardiomyopathy with persisting ECG abnormalities and cardiomegaly. Of these, three developed congestive cardiomyopathy and one developed constrictive cardiomyopathy. Twenty-two patients did not fall into the three functional types described by Goodwin4 and are referred to as "precongestive." The precongestive type comprised three distinct phases, an acute "arrhythmic phase," a quiescent "asymptomatic phase," and a stage of incipient cardiac insufficiency, the "early decompensated phase." The natural history of the disease is also discussed.
