Clinical aspects & pathomorphology of a polyradiculomyelitic form of herpes zoster

Resumo: On the basis of our own observation the clinical and pathomorphological phenomena in a zoster disease with polyradiculomyelitic progression are reported. 6 days after the zoster eruption, there was a caudal, progressing caudally advancing slack tetraparesis with a loss of reflex and a transversely limited sensory disturbance which finally reached the segment C 3 . The cerebrospinal fluid offered a moderate pleocytosis with increased albumen. After a few days, death occurred under the image of cardiac and circulatory failure in extended respiratory paralysis. The histological examination revealed a disintegration of the nerve cells in the spinal ganglia, demolding of the posterior and anterior roots as well as ganglional disease, perivascular lymphocyte infiltrates, glia growth and hemorrhages in the region of the hind horns, and rarely even the fore-horns. Furthermore, a patchy round cell infiltration of the meninges of the brain and spinal cord, an ependymitis granularis as well as subependymal perivascular infiltrations and glia growth could be observed. Based on observations by other authors, the specificities of the clinical and anatomical findings are exposed and the questions concerning the formal pathogenesis are discussed.
