Resumo: At autopsy of a patient with the Guillain-Barré syndrome dying 18 days after the onset of weakness, lesions of segmental demyelination were found in spinal nerve roots and peripheral nerves. Electron microscopy demonstrated vesicular breakdown of myelin associated with the presence of phagocytic cells beneath the Schwann cell basal lamina. In later lesions demyelinated axons were surrounded by Schwann cell cytoplasm. Some of these Schwann cells appeared damaged. The most acute lesions were in the most distal nerve examined. Axonal breakdown was rare. Remyelination was not seen.
